Welcome to the PHSA Spring Conference Registration process.

You may enter multiple participants on one registration and remit payment via
a consolidated check or credit card payment.

The first participant you complete a registration form for will be the primary registartion.
The address, telephone number, fax number and email address of the primary participant will
be used for all correspondence.

ONLINE REGISTRATION IS CLOSED
On-Site registration will be available at the conference.

Conference Details

The Conference Registration Due Date is April 9th, 2010. Registrations received after this date are assessed a late fee.

The Holiday Inn Harrisburg East Room Reservation Deadline is March 29th, 2010
Room Rate is $97.00 plus taxes. Call (717) 939-7841 to Reserve Overnight Accommodations

Cancellation Policy
In the case of a cancellation, PHSA must be notified by 5:00 PM on April 14, 2010.
Registration fees will be refunded if notice of cancellation is received PRIOR TO 5:00 PM on April 14, 2010.
Registration fees will not be refunded if notice of cancellation is received after 5:00 PM on April 14, 2010.
Registrants who neither cancel nor attend will be billed the full amount.
